Interview Document Format to Use for an Initial Interview with A Client
The best way to start an interview is with open-ended questions on the client’s bio-physical profile where his/her name, contact, gender and residential address are recorded followed by what made the client to visit, also known as the chief complaint/presenting problem. As recommended by the American Psychiatric Association (2016), the patient should be encouraged to tell his/her narrative without any interruptions.
